Collection begins with the April 2026 premium (withheld from pay in practice from May). The calculation method differs depending on your health insurance type.

Kyokai Kenpo, health insurance unions, and mutual aid associations (employer-based insurance)

A single nationally uniform levy rate is used (0.23% for FY2026).

  • Total annual levy = annual income (standard total remuneration) x 0.23%
  • Total monthly amount = annual total / 12 (rounded to the nearest yen)
  • Your share = total monthly amount / 2 (rounded to the nearest yen; the other half is paid by your employer)

This formula matches all five reference figures published by the Children and Families Agency (annual income ¥2M -> ¥192, ¥4M -> ¥384, ¥6M -> ¥575, ¥8M -> ¥767, ¥10M -> ¥959 per month).

National Health Insurance and the elderly medical system

These involve income-based reduction tiers for lower earners, and no simple rate formula has been published. This tool instead shows the closest published reference figure to your entered income, taken directly from the Children and Families Agency's tables. If your income falls outside the published range, the closest in-range figure is shown instead, with a note.

About the year-by-year trend

An income-based breakdown has only been published for FY2026, and only for employer-based insurance. For FY2027 and FY2028, this tool shows the system-wide average per enrollee (as published by the Children and Families Agency) as a reference trend, rather than an income-specific estimate. Actual individual amounts may change depending on future wage levels.

About the 'no net burden' statement
